Requesting Your Health Care Records 

To request copies of your records, please submit a written request using one of the options below:

• Client Portal: Medical Records Request or Treatment Summary Request Form

Please include:

• Your full name and date of birth
• The records you are requesting (type of record and date range)
• How you want the records delivered
• Your signature and the date
• If requesting records for someone else: documentation showing you are authorized to do so
